Personal plan. Coordination of health and/or social services

Description

You have the right to have a personal plan prepared if you have a long-term need for coordinated services from the public assistance authorities. The plan is designed to ensure that there is always someone who has the responsibility for follow-up. As a user, you are encouraged to actively participate in describing your needs for services, as well as your desires and objectives.
 

Target group

  • Individuals with a need for long-term and coordinated health and/or social services.
  • Participants in the qualifying programme conducted by the Norwegian Labour and Welfare Administration

Cooperating authorities

If you have a need for services from both the municipality and specialist health services (state), these have a duty to cooperate in preparing the plan.
 

Administrative procedure

Guidelines – applying for, or receiving the service

You or your relatives can take the initiative to have a personal plan drawn up, but the main responsibility lies with the assisting authorities.

Appendix/supplement

You must attach a power of attorney signed by you if someone applies on your behalf.

Administration time

The municipality will consider the application at the earliest opportunity. If the matter cannot be determined within one month. you will receive written information with the reason for this. At the same time you will be informed when a decision is assumed to be made.

Possibilities to appeal; procedure

The right of appeal is rooted in the individual Act (see section on Acts and guidelines). The county governor is the appeal body pursuant to the Social Services Act. The county health authority is the appeal body under the health legislation.

It is the right to the individual plan which may be appealed. But if an individual plan does not meet the requirements in the content (see regulations), you may also appeal this. The appeal is sent to that body in the municipality which has handled the case.
